Please read over this page to acquaint yourself with our eligibility requirement, hours of operations, and services provided.

Eligibility Criteria

To become a patient, you must first go through an eligibility screening process and qualify for services by meeting the 200% (or below) federal poverty level. Screenings are conducted Tuesdays through Thursdays by appointment.

You will need to bring:

  • Identification – Driver’s License or other official photo ID
  • Social Security Card (if available)
  • Proof of Pulaski County Residency
  • Proof of Income for the entire household
  • Letter showing Unemployment benefits, Disability Benefits, and/or Social Security Benefits
  • Food stamp award letter

Hours of Operation

  • Monday – Thursday 9am – 4:30pm, and Friday 9am – 2:00
  • Medical appointments are available on Tuesday, Wednesday and Thursday
  • New-Patient Screening is available Tuesday through Thursday by appointment
  • After screening, you may schedule an appointment.
  • We ask for a $2.00 donation for all medical appointments.
